Residential Services

Respite services are provided to children and adults with disabilities that reside at home. Funding is under the Home and Community Based Waiver Program. To become eligible for the Waiver program you must apply through your local County Case Management or DHS office.

Services are provided to a person in their home while the family needs time away. This service was designed to give families a break while ensuring that their family member is well cared for. This service is not available for a parent or guardian to use while they are at work. A family can access up to 576 respite hours a year.

Home & Vehicle Modification

To be eligible for this program a person with disabilities must currently be receiving services through the HCBS Waiver program. This program provides physical modifications to the home or vehicle to make it more accessible to the individual. There is a $5,000 lifetime maximum usage of this program.

Support Community Living

Supported Community Living (SCL) services were designed to support and provide skills and activities for daily living to adults and children with disabilities. Crossroads of Western Iowa supports people to become increasingly independent through a variety of training and living options. This leads to integration into a person’s community giving them the skills and knowledge to becoming as independent as possible. 

Services are based on a persons need and can range from one hour a month to 24 hours a day. Skills training encourage independence through cooking, housekeeping, personal hygiene, health, safety, laundry, budgeting, social skills, and others. A person can reside within their families home, in an apartment, or in small waiver group home settings.
